What is respiratory quotient?
Respiratory quotient is the ratio of carbon dioxide produced to oxygen consumed during cellular substrate oxidation. Pure carbohydrate oxidation has an RQ of about 1.00, while pure fat oxidation has an RQ of about 0.70; mixed diets and protein oxidation produce intermediate or context-dependent values.
Indirect calorimetry measures oxygen consumption and carbon dioxide production. Respiratory exchange ratio is measured at the mouth and may differ from cellular RQ during non-steady-state conditions [1].
Do you breathe out body fat as carbon dioxide?
The carbon atoms from oxidized triglyceride ultimately leave mainly as exhaled carbon dioxide, while hydrogen contributes to metabolic water. However, breathing is the exit route after oxidation; simply increasing breathing rate without greater metabolic demand does not cause meaningful fat oxidation and can produce dangerous hyperventilation.
The oxidation reaction also requires oxygen, and the energy released is transferred through metabolism rather than weighing the same as the lost tissue [2].
Can RQ prove that someone is losing body fat?
No, a low RQ or RER indicates a greater proportion of fat being oxidized at the measurement time, but it does not prove negative fat balance over the day. Net fat loss depends on fat intake, storage, oxidation, total energy balance, measurement conditions, and the time interval considered.
A person can oxidize fat between meals yet finish the day in neutral or positive fat balance.
